Περίληψη:Η παρούσα διδακτορική διατριβή διερευνά τους βιοϊατρικούς λόγους (discourses), τις ταξινομητικές τεχνικές, τις γραφειοκρατικές πρακτικές, τις διοικητικές στάσεις, τις πολιτισμικές αντιλήψεις αλλά και τις διαδικασίες υποκειμενικότητας, που ενέχονται στην αποτίμηση, τυποποίηση και μέτρηση της χρόνιας δυσφορίας στην Ελλάδα σήμερα. Η έρευνα εστίασε σε επιτροπές του Κεντρικού Συμβουλίου Υγείας (Κε.Σ.Υ.) του Υπουργείου Υγείας, οι οποίες είναι επιφορτισμένες με το έργο της αξιολόγησης αιτημάτων επείγουσας βιοϊατρικής παρέμβασης κυρίως σε ασφαλισμένους/ες ασθενείς βάσει «ενδείξεων»/ «κλινικών κατευθυντηρίων οδηγιών»/ «κλινικών πρωτοκόλλων», ενώ ως συγκριτικό εθνογραφικό παράδειγμα επιλέχθηκε το πεδίο των υγειονομικών επιτροπών πιστοποίησης της αναπηρίας. Η «μεγάλη αφήγηση» της «ιατρικής βασισμένης σε ενδείξεις» (evidence-based medicine) και της παραγωγής κλινικών κατευθυντηρίων οδηγιών (clinical guidelines) ή κλινικών πρωτοκόλλων (clinical protocols) που κατευθύνουν την κλινική απόφαση με βάση δεδομένα της βιοϊατρικής στατιστικής επιστημολογίας, αποτέλεσε κυρίως από τη δεκαετία του 1990 κομβικό σημείο αλλαγής παραδείγματος στην κλινική πρακτική. Στα συμφραζόμενα μιας νεοφιλελεύθερης καμπής της κρατικής πολιτικής πρόνοιας και ασφάλισης, όπου η θεραπευτική σχέση αποτιμάται με όρους «κόστους/οφέλους», εξετάζω πώς οι ανομοιογενείς και ενίοτε αντιφατικές «επιστημονικές ενδείξεις» της κλινικής επιδημιολογίας μετατρέπονται από συμβουλευτική βιβλιογραφία σε υποχρεωτική νόρμα της κλινικής απόφασης, συγκροτώντας ένα βιοϊατρικό-γραφειοκρατικό δίκτυο αξιολόγησης της πρόσβασης στις υπηρεσίες κλινικής φροντίδας. Έτσι, μελετώ πώς αποτυπώνονται σε καθημερινό επίπεδο οι όροι της «ιατρικής αποτελεσματικότητας» (effectiveness) και «οικονομικής αποδοτικότητας» (efficiency) στην κατάσταση υγείας των ασθενών, όταν αυτοί/ες έρχονται αντιμέτωποι/ες με ένα σύνθετο και ετερογενές βιοϊατρικό-διοικητικό δίκτυο ειδικών και μη. Ιχνηλατώντας σχέσεις εξουσίας και υποκειμενικότητες που συμμετέχουν στη βιοπολιτική της γραφειοκρατικοποίησης του πόνου, φωτίζω τους τρόπους με τους οποίους παράγονται επισφάλεια, αποκλεισμοί και ανισότητες, παρά την ιστορική, φιλελεύθερη υπόσχεση ασφάλειας, λογοδοσίας και καθολικής πρόσβασης σε ποιοτικές παροχές βιοϊατρικής φροντίδας. Επιχειρώ να αναδείξω τις πολιτισμικές πρακτικές που διέπουν και στηρίζουν αυτή την παραδοξότητα, έτσι ώστε να διανοιχθεί χώρος για μια διαφορετική, μέχρι τώρα «α-διανόητη» μορφή ανακούφισης της κοινωνικής οδύνης.
Abstract:In my dissertation I try to examine how the bureaucratic field of a public service such as the Greek Ministry of Health and Social Solidarity is a material and relational space when it becomes a major site of social suffering for the patients who experience and manage their illness through their everyday encounters with the state. In order to understand how the pain of the suffering body is the daily object of a whole bureaucratic network, I focus on how the limits, the restrictions, and constraints for care giving affect the illness experience. The importance of this ethnographic position is that it engages with the numerous practices based on evidence based medicine (guidelines) in which the state is revealed and becomes inscribed in persons and their relations. I laso argue that the aspect of time, as it is differently involved in bureaucratic processes and in the patient life story, plays a major role in this complicated bureacratic process. Therefore, I put a special emphasis on these patient experiences of enforced dependency on the doctors decision and on the administrators who manage the urgent health demands. Sometimes patients seek to negotiate with the administrators the whole process in order to go round these time-consuming procedures and get the decision as soon as possible. Also sometimes, the negative desicion forces the patients to find other alternatives to make the exam. In the above described context, I explore the terms in which the patients call for care is approved or rejected and how care and suffering are interlinked in the complicated and precarious realm of liberal and neoliberal welfare. More specifically, I try to illustrate that social suffering is not an exception or a paradox vis-à-vis welfare, but rather a constitutive element of the welfare state, which in everyday practice works by regulating, classifying, determining and deciding who deserves ‘effective care’ and who does not. So in my dissertation, I trace this dense network of claims, discourses and practices enacted by patients, doctors, and administrators, who participate –albeit differently and asymmetrically- in the management of human vulnerability. This amalgamation of expectation, hope, despair, claims, cure, empathy, indifference, medical discourse, welfare administration, and institutional limitations, is what I call here ‘bureaucratisation of pain’. As my focus is on everyday practices of the medical bureaucratic apparatus, I raise the following questions related to the institutional management of vulnerability in the context of a welfare state : How is social suffering produced and administered through the bureaucratic management of the illness? How does this bureaucratic structure break to pieces the suffering body and how does it transform the human experience into a bureaucratized object and even a standardized commodity?
